An autonomous AI plugin (Claude) running on a cron loop tried to append an entry to an existing markdown file but used a write operation that overwrote and deleted important historical data. The issue was that the model chose to use 'Write' which replaces the file instead of 'Edit' which allows appending, causing critical data loss without user confirmation.
